Urgent Care for Children uses the Alabama High School Association form for school sports physicals, while each camp typically has their own. Make sure to bring the needed form with you to the office! During the visit, your provider will ask for medical history and then do a complete exam to ensure your child is healthy and ready to participate safely in their activities. It’s best to come in two or three days before you need your physical form to give the staff sufficient time to complete the form.
How long does a physical take?
A physical typically only takes 10-15 minutes once you’re in the room.
Is an appointment necessary? How can someone schedule an appointment?
An appointment is not necessary, but you can “get in line” ahead of time on the website.
Can a patient get multiple copies if they need a physical for more than one activity?
